What to Bring
Forms
- If you have completed any patient forms, please bring them with you.
HMO Patient's Referrals
- Bring your referral! If you are a member of an HMO, please remember that it is your responsibility to keep track of and arrange for referrals for your visit to this office. Your HMO will not cover charges for your visit unless a proper referral is received in our office by the time the physician sees you. If a referral from your primary care physician is not received in a timely fashion, then your appointment will be delayed or need to be rescheduled for a later date.
Your Insurance Card
- Don't forget to bring your insurance card! Since we submit claims for you to participating insurance plans, we will need to see your insurance card to gather the proper infromation.
X-Rays, Scans, CT or MRI Films
- If you have had any x-rays or scans (CT or MRI) related to your condition, please bring the films with you to your appointment. Radiology offices will only send or fax reports and your physician can assess your problem better by viewing the actual pictures. If there are any pertinent records, they should be brought in as well.
